What is KSA
The abilities and characteristics that enable a job holder to accomplish the goals of the task statement
What is a threat?
A threat is a external event that occurs beyond the influence of the flight crew thus increasing operational complexity, which must be managed
Break down a threat into three parts
Anticipated, unexpected, latent
What are the three types of errors
Procedural error, communication error, handling error
What is an undesired aircraft status
A misapplication by the crew that leads to a deviation in safety
What is a hard countermeasure
A countermeasure that is in place before the flight
Break down a the soft countermeasures
Avoid; requires extreme situational awareness to foresee the problem, trap; see the immediate error and deal with it so that is no consequences, mitigate; if the threat is not trapped mitigate to reduce harm
Break down the shell model
Software; paper stuff,hardware; bolted onto airframe, environment; flying environment , live ware; human-human element, central live ware; yourself as pilots are central
What is the Swiss cheese model
A belief that an error doesn’t just happen without a lot of problems adding together
What is safety culture
The culture within an organisation or country that is defined by actions in relation to safety
What is the difference between open and closed culture
An open culture means you are happy to report and have other benefit off of it however a closed culture means you will not see the point
