* favor disease over artifact in a high caries risk
differentials
1) sinuses may be asymmetrical
- sinus pneumatize to level of alveolar crest
- however, they do not pneumatize anteriorly
2) could be radicular cyst
3) glandular odontogenic cyst (not on differential)
- nice corticated boundary
- benign and aggressive
4) refer them to a pathologist
- CBCT scan before biopsy
- need to see entire periphery of the lesion
label teeth and recognize what is missing
1) always count backwards from what you know
