What is the Francis report?

A
  • Inquiry into the failings at Mid - Staffordshire, Robert Francis published a report on 6th February 2010 with recommendations
  • Poor standard of care in Mid Staf made national headlines
  • Estimated that 400-1200 ppl may have died unnecessarily between 2005 -2008
  • Understaffing and money saving system, focussing on meeting targets over patient care
  • Francis report recommended - staffing levels, protection of whistle blowers, training staffs

What is revalidation?

A

Revalidation is the process for doctors to positively affirm to the GMC that they are up to date and fit to practice. It applies to all doctors usually every 5 years by having regular appraisals.

What is the Nicholson challenge?

A

David Nicholson, previous NHS CHief executive demanded for a NHS 20 billion pounds efficiency savings by 2015. This is one of the largest NHS budget cut. 2012 BBC article stated that 12 NHS Trust were already not financially viable to withstand the pressure of current cost cutting measures and goals demanded by Mr Nicholson.

Tell me about the WHO checklist.

A

WHO surgical safety checklist to ensure that all conditions are optimum for patient safety, minimising the most common and avoidable risks in surgical patients.
3 stages,
Before induction of anaesthesia, confirm ID, site, what procedure, blood loss, consent, allergy and anaes checklist
Time out, introduction by team, confirm ID, procedure, abx, allergy, xrays, anaes/surg concerns
Sign out, instrument check, equipment and specimen check. Anaes and surg key concerns and post op management.

Tell me about the shape of training review.

A

A report written by Professor Greenway regarding to postgraduate medical training. It focuses on ensuring training is responsive to changing demographics and patient needs. However from the surgical perspective, surgery takes time to master. It is unclear how without elongating training, surgeons who are highly specialised would acquire the required skills and experience in this proposed broad based training. With regards to rewarding registration after 1 year, there are some concerns on the lack of a national curriculum across medical schools. Medical schools will have to provide assurance that graduates are capable of working safety in a clinical environment having had limited experience on hospital wards.
