What do the Reed-Sternberg cells make? What does this cause?
Secrete cytokines.
- B symptoms - fever, chills, weight loss
- Attract reactive lymphocytes, plasma cells, macrophages, eosinophils
- May lead to fibrosis
What is Hodgkin’s lymphoma? What would you see on histology? What markers are positive for it?
Neoplastic proliferation of Reed-Sternberg cells which are B cells with multi-lobed nuclei and prominent nucleoli “owl eyed”. Positive for CD15 and CD30.
What are the subtypes of Hodgkin’s lymphoma?
Nodular sclerosis
Lymphocyte-rich - best prognosis
Mixed cellularity - abundant eosinophils
Lymphocyte depleted - most aggressive, seen in elders or HIV + pts
A 27 yo pt comes to your obgyn clinic for annual checkup. Physical exam shows an enlarging cervical lymph node. What do you think she has? What would biopsy show?
She has nodular sclerosis type of Hodgkin’s lymphoma which is the most common type. Lymph node is divided by bands of sclerosis. Reed-Sternberg cells are present in lake-like spaces.
