transient synovitis vs. septic arthritis
similar early Sx: progressive hip, groin, thigh pain limp or inability to weight bear fever irritability
due to the overlap, there are no absolute criteria for the diagnosis of either
transient synovitis
acute onset followed by spontaneous recovery and no systemic upset
benign condition, with a small amount of fluid in the joint
most common cause of hip pain in children 3-10yo (peak at 5-6)
more common in boys
often preceded by viral infection
self-limiting, no long term sequelae
SUFE
displacement of the proximal femoral epiphysis off the femoral neck
usually aged 11-14
more common in obese and boys
bilateral in 20-40%
hip often held in abduction and external rotation
down’s syndrome
5-8% will develop abnormalities of the hip
most common condition is subluxation, sometimes ass. w/ malformation of the acetabulum
perthe’s
idiopathic AVN of the femoral epiphysis
usually aged 4-10yo, peaking 5-7yo
boys more than girls, bilateral in 10%
