Hip Goniometry and Special Tests Flashcards
Flexion
0-120 norm
Fulcrum: Greater Trochanter
Proximal Arm: Lateral Midline of Pelvis
Distal Arm: Lateral Epicondyle
Extension
0-10 norm
Fulcrum: Greater Trochanter
Proximal Arm: Lateral Midline of Pelvis
Distal Arm: Midline of femur/patella
Abduction
0-45 norm
Fulcrum: ASIS
Proximal Arm: Contralateral ASIS
Distal Arm: Midline of femur/patella
Adduction
0-30 norm
Fulcrum: ASIS
Proximal Arm: Contralateral ASIS
Distal Arm: Midline of femur/patella
Internal Rotation
0-45 norm
Fulcrum: Patella
Proximal Arm: Perpendicular to floor
Distal Arm: Midline of lower leg between malleoli
External Rotation
0-45 norm
Fulcrum: Patella
Proximal Arm: Perpendicular to floor
Distal Arm: Midline of lower leg between malleoli
Trendelenburg Test
pt slowly march in place, can add ankle PF (If no PF, S1)
Look at pelvis
-Greater than 1/2” depression is glute med weakness, hip compression intolerance, or S1 myotome involvement
Faber/Patrick’s/Figure 4 Test
pt foot placed opposite distal thigh proximal to knee in supine
PT controls abduct/ER hip
-Hypomobile SI or hip joint, tight adductors
Symptoms at PSIS: SI/Lumbar joint pathology
Symptoms at GT/Thigh: hip joint pathology
Symptoms at knee: Meniscal pathology
Thomas Test/3 Muscle Kendall Test
pt supine with ipsi. knee off plinth, contra knee to chest
Hip flexed: Tight IP
Knee Flexed
Craig’s Test
pt prone with knee flexed to 90, passively IR/ER until GT is parallel to supporting surface
-Measure angle of tibia from vertical
15 degrees is anteversion
FAIR Test
pt. sidelying hip flexed to 45 and knee to 90 allow hip to adduct to table PT IRs hip toward end range Palpate Piriformis Positive if pain at intersection of sciatic and piriformis
Ober test
pt sidelying with supportive LE flexed to 45 and knee to 90
Tested hip brought to neutral extension with knee flexed to 90, hip abducted
Allow hip to adduct while stabilizing pelvis
Measure at point that pelvis begins to depress Inclinometer over lateral femoral condyle
Adduction
Scour Test
pt supine, flex hip to 90 with slight adduction. Scour 2x without compression. If no symptoms, scour 2x with compression
Reproduction of symptoms could mean intra-articular pathology
If pain with scour, resisted hip flexion for 5 seconds
Flexion-Adduction Test
pt supine, hip flexed to 90
PT adducts hip from zone 3 towards zone 1
Assess for firm end feel
Tests for non-specific hip pathology
