Slump Test
Pt sitting legs together and back of knees against table
Slump as much as possible
Maintain full spinal flexion with overpressure, ask patient to look down
Ask patient to extend knee
Add dorsiflexion to ankle
Neck flexion is released to see if symptoms abate
Straight Leg Raise
pt supine on table with head and neck in neutral
Support at heel and raise leg to point of symptom reproduction
Add DF at point of reproduction
Prone Knee Bend
Pt prone on table
Examiner places hand on pelvis, flexes pts knee
Bend knee until reproduction of symptoms
Sensitizing movements include PF, DF, hip Ext, head movements
Prone Instability Test
Pt lies with torso on table, legs off edge, feet on floor
Examiner performs A:P over spine looking for pain
Maintain contact with painful segment, pt lift feet off
Perform P:A with feet off
Positive if significant reduction of symptoms
Quadrant Test
pt standing with feet shoulder width apart
Examiner asks patient to lean back, rotate, and side bend to one side
Movement is repeated on opposite side
Positive for reproduction of pt’s concordant sign
Distraction (Gapping)
Pt supine on table, establish baseline
Locate ASIS, cross arms, place palms over ASIS
Apply a lateral-posterior force slowly taking up slack
Hold this for 30 seconds, reestablish baseline
Apply a thrust
Positive if produces pain in SI
Compression Test
Pt in side-lying, painful side up, get baseline
Cup iliac crest and apply a downward force through the ilium, take up slack, hold for 30 seconds
Apply a thrust to produce symptoms
Positive if reproduction of concordant sign
Thigh Thrust (Femoral Shear)
Patient supine near edge of table Examiner opposite painful side Flex hip on painful side to 90° Place hand under the sacrum Appy a downward thrust through femur Positive is pain posterior to hip or near SI on thrust side
Pelvic Torsion (Gaenslen’s)
Pt supine with painful leg off table, assess baseline Raise non painful leg to 90 Downward force applied to painful leg Can be performed up to six times Repeat procedure on opposite side Positive if concordant sign
Sacral Thrust
Pt prone
Palpate sacrum and place one hand on sacrum, other over the application hand
Apply a P:A force and assess response
Repeat up to 6 times to attempt to provoke
Positive if reproduction of concordant sign
Standing flexion test
Pt standing Examiner palpates PSIS bilaterally Pt flex forward Evaluate movement Positive if asymmetrical movement Repeat at inferior angle of sacrum
Seated Flexion test
Same as standing flexion except seated
Supine to sit test
Pt supine in hooklying position
Perform barstow-weber maneuver
Assess leg lengths
Pt sits up
Assess leg lengths
If one leg moves further than the other the test is positive
Short to long is posterior rotated inominate
Long to short is anterior rotated inominate
Gillet test
Examiner behind standing pt palpate PSIS
Patient lifts hip to 90°
Examiner looks for same side PSIS drop (Normal) or rotates superiorly
Positive if it does not drop or rotate superiorly
