Hip Flexion Grades 0-2
Muscle: iliopsoas
Position: side lying on side being tested, flexion of hip (knee to chest)
Stabilization: support under non-test limb, holding underneath knee and lower leg
Hip Flexion Grades 0-2 (alternative)
non weight bearing on test leg
Muscle: iliopsoas
Position:
Grade 2: side lying on side NOT being tested, hip flexion with extended knee
Grades 0-1: supine, 90 degrees knee flexion, flexion of hip
Stabilization: support test limb, holding underneath knee and lower leg
Hip Flexion Grades 3-5
Muscle: iliopsoas
Position: sitting on edge of table (can hold onto sides of table), flexion of hip
Stabilization: ipsilateral iliac crest (making sure pt is not leaning back/moving pelvis)
Resistance: anterior, distal femur
Hip Extension Grades 0-2
Muscle:
- gluteus maximus
- biceps femoris
- semitendinous
- semimembranosus
Position: side lying on side being tested, extension of hip (knee extended)
Stabilization: support upper non-test limb, holding underneath the inner thigh, knee and lower leg
Hip Extension Grades 0-2 (alternative)

Position:
Grade 2: side lying on side NOT being tested, hip extension with extended knee
Grades 0-1: prone, extension of hip
Stabilization:
Grade 2: support test limb, holding underneath knee and
lower leg
Grades 0-1: none
Hip Extension Grades 0-2 (gluteus maximus bias)
Muscle: gluteus maximus
Position: side lying on side being tested, extension of hip (knee flexed)
Stabilization:
- support upper non-test limb, holding underneath the inner thigh, knee and lower leg
- pelvis/gluteus maximus
Hip Extension Grades 3-5
Muscles:
- gluteus maximus
- biceps femoris
- semitendinous
- semimembranosus
Position: prone, extension of hip (knee extended)
Stabilization: ipsilateral posterior iliac crest of the pelvis (no lumbar extension)
Resistance: posterior aspect of distal femur
Hip Extension Grades 3-5 (alternative)
*tight hip flexors* Muscles: 1. gluteus maximus 2. biceps femoris 3. semitendinous 4. semimembranosus
Position: prone with lower half of body off table, feet on ground (pillow under trunk), lift leg into extension
Stabilization: ipsilateral posterior superior portion of pelvis
Resistance: proximal to posterior knee (above popliteal fossa)
Hip Abduction Grades 0-2
Muscles: gluteus medius and gluteus minimus
Position: supine, slide leg into abduction (toes pointing up)
Stabilization: ipsilateral lateral pelvis
Hip Abduction Grades 3-5
Muscles: gluteus medius and gluteus minimus
Position: side lying on side NOT being tested (non-test leg in 90 degrees flexion), abduction of hip (knee extension)
Stabilization: ipsilateral iliac crest
Resistance: lateral/distal thigh, proximal to medial aspect of knee
Hip Adduction Grades 0-2
Muscles:
- adductor longus/brevis/magnus
- pectineus
- gracilis
Position: supine, slide leg into adduction (toes pointing up)
Stabilization: ipsilateral lateral pelvis
Hip Adduction Grades 3-5
Muscles:
- adductor longus/brevis/magnus
- pectineus
- gracilis
Position: side lying on side being tested (non-test leg abducted in air with slight knee flexion, supported by PT), adduction of hip (knee extended), hold in air
Stabilization: support non-test limb underneath inner thigh (30 degrees hip abduction) while also stabilizing anterior pelvis with elbow
Resistance: medial/distal thigh, proximal to medial aspect of knee
Hip Internal Rotation Grades 0-2
Muscles:
- TFL
- Gluteus maximus
- Gluteus minimus
Position: supine, rotate leg inward
Stabilization: anterior ipsilateral pelvis (prevent rocking)
Hip Internal Rotation Grades 3-5
Muscles:
- TFL
- Gluteus maximus
- Gluteus minimus
Position: sitting on edge of table (legs hanging off), foot moves laterally (internal rotation of hip)
Stabilization: medial aspect of knee
Resistance: lateral malleolus
Hip External Rotation Grades 0-2
Muscles:
- piriformis
- superior/inferior gemellus
- obturator internus/externus
- rectus femoris
Position: supine, rotate leg outward
Stabilization: anterior ipsilateral pelvis (prevent rocking)
