Q

What actions are taken if chemistry sample indicated specific activity exceeds 0.1 uCi/gm?

A

Refer to TS 3.4.12, attempt to restore < limit in 48 hours, if not restored (< 0.1 or dose equivalent Iodine > 6 uCi/gm) then enter mode 3 in 6 hours at 30%/hr and cooldown to mode 5 in 36 hours.

Q

What is the recommended configuration of letdown during high gross gamma activity section?

A

Maximize letdown at CRS discretion

or

Isolate Letdown if vital area access is jeopardized due to dose rates

6
Q

What actions are taken if letdown is isolated?

A

Maintain < 290” by reducing seal injection flow by placing CV-1206 in override and maintain minimum of 2.5 gpm SI to each RCP or isolate SI and ensure ICW cooling.

Q

What actions are taken for a circ water leak?

A

If leakage associated with single waterbox, then lower power < 99.75% and SG/RX demand in hand, Close condenser vacuum isolation, Close trap outlet isolation (WBV), Open Water box vent and close water box inlet.

If CW pumps must be secured soon as possible then trip RX (otherwise RPSD), actuate EFW, trip MFPs, Open ADV isolations, Close MSIVs and trip all CW pumps. Vacuum Pumps in PTL, Break vacuum and place condensate pumps in PTL.

Q

What actions are taken for a condensate/feedwater leak?

A

If hotwell stable and downstream of MFPs and threat to operation, trip rx, actuate EFW and trip MFPs.

If hotwell stable and leakage from E-3/4/5/6 or 8 then perform RPSD to lower power in preps for bypassing heater.

Q

What actions are taken for a SW leak?

A

Close SW to ICW coolers from loop with lowest pressure.

If un-isolable leak > 25 gpm in non-seismic boundary of ICW cooler and aligned to ECP then isolate ICW cooler within 12 hours.
