What is HSV?
Herpes simplex virus multiplies in epithelial cells of mucosal surfaces, giving vesicles and ulcers. Lifelong infection
what are types of HSV
HSV 1: tends to cause herpes labialis
HSV2: tends to cause genital herpes
What are Sx of herpes?
Labialis: tingling and burning around mouth, development of vesicular then ulcerative lesions in oropharynx/perioral mucosa
Genital: asymptomatic, or tingling and burning, recurrent genital ulcerations, dysuria
What are risk factors for HSv
HIV
immunocompromised
What Ix in HSV?
Viral culture from swab
PCR
What Rx for HSV?
Acyclovir, increase dose if recurrence, long term fo suppression
What are complications of HSV?
Erythema multiforme
Keratitis
meningitis
encephalitis
What is HZV?
reactivation of varicella zoster (chickenpox), causing shingles
How does HZV reactivat
stays dormant in dorsal root ganglion
What are Sx of HZV?
Burning and stabbing pain following by vesicular rash with itch in affected dermatome
What are risk factors for HZV
Age >50, steroids, chemo, malignancy, hiv
What Ix for hzv
Clinical, but can do PCr
What ddx fo HZV
contact dermatitis, ulcerative keratitis, trigeminal neuralgia
what rx for hzv
aciclovir
What are complications of HZv
HZ ophthalmicus: if in trigeminal nerve, get eye Sx and eye ointment
