Malaria and HIV
FBC
Blood cultures
LFTs
What is traveller’s diarrhoea?
Traveller’s diarrhoea (TD) is defined as ≥3 unformed stools in 24 hours accompanied by at least 1 of the following:
• Fever
• Nausea
• Vomiting
• Cramps
• Tenesmus
Bloody stools (dysentery) during a trip abroad, typically to a low- or middle-income country
What Ix for traveler’s diarrhoea?
tool culture and sensitivity • Stool occult blood • DRE • Multi-pathogen molecular diagnostic PCR • Stool ova and parasite
What is enteric fever?
Systemic symptoms cause by salmonella typhi and paratyphi
How is enteric fever/typhoid transmitted
Faceal oral route, humans only
What are Sx of enteric fever?
Diarrhoea or constipation
Abdominal pain
Bradycardia
Rose spots
Headache, fever, arthralgia
Encephalopathy
How is enteric fever diagnosed?
Blood and stool cultures
Serology (not great)
What areas are enteric fever common in?
Africa, Asia, South amercia and caribbean
How is enteric fever treated?
Azithromycin +/- IV ceftriaxone
normally resistance to quinolones
Fluids, antipyretic, nutrition
What are complications of enteric fever?
Osteomyelitis, GI bleed, meningitis
What is dengue fever?
Viral infection transmitted by Aedes mosquito biting during day.
What are Sx of Dengue?
Breakbone fever: retro-orbital headache, fever, myalgia, arthralgia, maculopapular rash, cough, facial flushing
What Ix in Dengue?
PCR for virus or serology
Bloods: low platelets, low WCC, transaminitis
What is Rx for dengue
Supportive; fluids,
What is viral haemorrhagic fever?
Complication of dengue/yellow fever, involving increased vascular permeability, low platelets, bleeding
May need transfusion
