Transfusion Reaction: Initial Signs
Fever, chills, back pain, dyspnea, hypotension, dark urine.

Rh incompatibility between mother and fetus, leading to RBC destruction in the fetus.
Transfusion Reaction: Immediate Nursing Actions
Stop transfusion, maintain IV access with saline, notify physician, monitor vitals, send blood back to the lab for analysis.

Administer Rhogam to Rh-negative mothers at 28 weeks gestation and within 72 hours postpartum if baby is Rh-positive.
