Q

What is the mechanism by which massive packed RBC transfusion (eg. after trauma) can cause paresthesias?

A

Whole blood is mixed with anticoagulant that contains citrate. The packed RBCs from these preparations also contain citrate, which binds to Ca2+ after infusion into the patient -> hypocalcemia -> paresthesias.

See sketchy regarding citrate chelation in renal tubules for prevention of calcium stones.
