Q

Explain S3

A

Abnormal extra heart sound
Occurs when ventricles are resistant to filling during early rapid filling phase (protodiastole)

Late sound

Immediately after S2, when AV valves open and atrial blood first pours into ventricles

“Kentucky”

Q

Explain S4

A

Abnormal extra heart sound
Occurs at end of diastole (presystole) when ventricles are resistant to filling

Atria contract and push blood into noncompliant ventricle

Vibrations

Just before S1-Early sound

“Tennessee”

Explain the murmur scale

A

1/6: Can’t hear it- use echocardiogram

2/6: Can hear if room is completely silent

3/6: Can hear faintly

4/6: Louder

16
Q

What are 2 common medicinal treatments for chest pain/heart attacks?

A

Baby aspirin: chewed up for faster absorption, keeps blood cells from sticking together

Nitroglycerine: on/under tongue, dilates coronary arteries

17
Q

How is fatigue related to heart attacks/heart disease?

A

Decreased blood flow to brain

18
Q

Define:
Orthopnea
Nocturia

A

Orthopnea: can’t breathe when laying down- ask how many pillows they sleep with

Nocturia: getting up to pee at night

19
Q

What are some personal habits that affect cardiac risk?

A

Nutrition
Smoking
Alcohol
Exercise
Drugs/medications

20
Q

What kind of tone is best when asking questions about alcohol/nutrition/smoking?

A

Flat affect- non-reactive or judgemental

21
Q

What position is best for carotid artery evaluation?

22
Q

What position is best for assessing jugular veins and precordium?

A

Supine with head and chest slightly elevated 30-45 degrees

23
Q

Explain JVD assessment

A

Jugular Vein Distention- blood buildup causing a visible pulsation/bulging

24
Q

Explain apical impulses

A

Pulsation on anterior chest from left ventricle rotating against chest wall during systole

not always visible

25
How is apical impulse palpated?
Localize w/ one finger pad Patient exhales and holds it
26
What is a normal finding for palpation across the apex, left sternal border, and base?
Normally no pulsations occur
27
What are the 4 valve areas for precordium auscultation?
Aortic valve: second right interspace Pulmonic valve: second left interspace Tricuspid valve area: left lower sternal border Mitral valve: Fifth interspace around left midclavicular line
28
What is ERB's point?
Left 3rd intercostal space to feel S1 and S2
29
What is the acronym for the sites of precordium auscultation?
All: Aortic valve People: Pulmonic valve Enjoy: Erb's point Time: Tricuspid Magazine: Mitral
30
What needs to be done to assess for murmurs?
Use bell of stethoscope in the same 5 spots
31
Explain thrills vs. heaves
