Which two drugs form the basis of treatment for all heart failure patients with heart failure due to LVSD?
An ACEi with a beta-blocker.
If patients with LVSD have been treated with an ACEi and a beta-blocker and are still symptomatic, what can they be treated with?
An aldosterone antagonist.
Give examples of aldosterone antagonists.
Spironolactone, eplerenone.
In heart failure, what circumstance is digoxin reserved for?
Worsening or severe heart failure despite other treatment.
Which drugs should be given to patients with heart failure who are also experiencing fluid overload?
Loop or thiazide diuretic (with salt or fluid restrictions where appropriate).
When potassium sparing diuretics are used with ACEis or ARBs, what may occur? What should be avoided in this situation?
Severe hyperkalaemia. Potassium supplements should be avoided.
If an ARB is used as an adjunct to an ACEi, what is there an increased risk of?
Hyperkalaemia, hypotension, renal impairment.
