How to approach quitting alcohol w pt?

A

Outline risks to the patient and make them aware of the recommended amount.

Ask them how they feel about the amount they are drinking? Would they like to reduce the amount?
Do they think they will be able to do it? This can ascertain where in the cycle of change they are.

Other services- turning point, AA, If excessive drinker, ( 50 or more units a day) do not quit cold turkey, risk of delerium tremens and death.

Prescribe thiamine and B12

A

how many pack years? would they like to stop smoking? where on cycle of change are they?

Education on harm reducation methods.

Vaping- no tar or CO, not been around v long but better than smoking. Nicotine patched- can get these on prescription.

A

Do strengthening activities that work all the major muscle groups (legs, hips, back, abdomen, chest, shoulders and arms) on at least 2 days a week

Do at least 150 minutes of moderate intensity activity a week or 75 minutes of vigorous intensity activity a week

Spread exercise evenly over 4 to 5 days a week, or every day

Reduce time spent sitting or lying down and break up long periods of not moving with some activity

How to approach incorporating diet changes and exercise into lifestyle?

A

Be mindful as it can be sensitive.

Determine whether other cause of increased weight, birth control, PCOS?, thyroid problem. When these are ruled out, advise of changes that can be made e.g:

  • having lunchs packed, less cakes, processed foods etc.
  • try to get moving more, social prescribing? sports that you may enjoy, walking? the gym?
