What is a Wound?
A break in the skin or mucous membrane & becomes a portal of entry for microbes
What is a Pressure Ulcer/Sore?
A wound that develops when the skin is damaged from prolonged pressure
Causes of Pressure Ulcers?
- Pressure
- Friction
- Shearing
- Breaks in the skin
- Poor circulation to an area
- Moisture (perspiration, urine, stool)
- Dry, flaky skin
What are the risks for developing a Pressure Ulcer?
- Limited mobility
- Loss of bowel or bladder control
- Poor nutrition
- Reduced sensation
- Circulatory problems
What are the roles of a HCA in terms of Preventing / Treating a Pressure Ulcer?
Follow repositioning schedule
Keep linens clean dry and wrinkle free
Provide good skin care
Proper lifting and transfering
Report any signs or skin breakdown
Describe the different types of Wound Drainage
Serious drainage: clear watery fluid
Sanguineous drainage: bloody drainage
Serosanguineous drainage: thin, water drainage
Purulent drainage: thick, green, yellow or brown drainage with foul odour
