Respite care
Person living in a home that needs home health care on occasions
It gives the primary caregiver help and a break
Nurses can give referrals for those patients families who may express they will need help at home to care for there ill family members
Managed patient care
Focus on health promotion, client education, early identification of disease, and use of health care resources
What is the focus of patient care
Assessment and development of a plan of care. Coordination of all sources, referrals and follow up
Main focus (goal) of nurses to patients
Assessment, treatment planning, help client obtain information about resources to help, advocacy and MONITORING
Whose does Medicaid cover
People over 65
Certain young people with disabilities
People with end stage renal disease requiring dialysis or renal transplant
Hospice care
When doctors state patients have 6 months or less to live
Palliative and supportive care: physical, psychological, social and spiritual care for the dying
Focus on quality of life and preserving patient dignity rather than prolonging the length
Hospice nurses can continue to help families with bereavement up to 1 year after one has passed.
Help them with the grief process

Disease is incurable, not dying soon. But to make quality of life comfortable
Used in conjunction with medical treatment
Used in all health care settings
Primary healthcare
Common health problems:
Diabetes, arthritis, depression, hypotension and any PREVENTATIVE measures - vaccination, mammogram
Prenatal, family planning, immunization, preventative health screening for all people, health education
Focus on :
Weight loss, diet, exercise, reduce alcohol intake, immunization, safe sex practice, effective parenting, seat belts, car seats, water treatments
Secondary health care
Screenings for specific diseases especially in older adults.
If patients have health concerns and they get screened or see specialist it is considered secondary
Focus on early screening for early detection of diseases
Disease identification and management
Screenings for :
Blood pressure, cholesterol, pap, mammogram, testicular, glaucoma, hiv, skin cancer, family counseling
Teritiary healh care
Management of rare and complex disease and disorders
Subspecialist, surgeons
Medication, medical therapy, surgical treatment, rehabilitation, physical therapy, occupational therapy, job training
Physiological needs
Must be met first
Physical activity, rest, food, life, water, elimination, oxygen, temperature, sexuality, shelter
Breathing, circulation, temperature, intake of food and fluid, Elimination of waste, movement
Safety and security needs 2nd level
Protection from potential or actual harm
Building trusting relationship with patients
Housing, community, neighborhood, climate
Love and belonging 3rd level
Giving and receiving love, feeling of belonging
If unmet patient will feel lonely and isolated
As a nurse to implement this; include family and friends in the care of patient, establish trust with patient and mutual understanding, encourage patient to communicate with the nurse, referring to specific support groups
4th level self esteem needs
Persons need to feel good about themselves
Nurses can respect the patient values and beliefs, encourage patient to set attainable goals and to facilitate support from family or significant other.
Self actualization needs 5th
Nurses helps to focus on the persons strength and possibilities rather than problems, help patient with a sense of direction and hope to maximize full potential
