Discharge teaching to a patient
Use tech back method to ensure client understands
Encourage client to ask questions
Give instructions to client on how to administer their prescribed medications
Educate on adverse side effects and when to contact PCP
Explain how to perform prescription treatments and any special equipment needed
Help give ways they can access community resources
If needed go over schedule of home care services
Discuss dietary and activity measures
Discuss any complications that could occur and when they should follow up with provider
