Tension headache location
Most often:
Band around mid head
Less often: all over head
Migraine headache location
One sided
Most often: Temple
Less often: whole side of head
Cluster headache location
Focused on one eye
Tension type headaches
Similar to migraines
Evolve from episodic to chronic
Constant dull pressure around band
Assoc. w/ neck pain, photophobia, phonophobia
Dx by Hx, EMG (electromyography),
Migraine headaches
Risk factors:
Obesity, female, depression
With aura: CLASSIC
Without aura: COMMON
throbbing pain unilaterally
Migraine drugs
Moderate to severe:
Triptans are first line- vasoconstriction, reduce inflammation
Preventative:
- Antiseizure
- beta blockers
- Botox
Cluster headache
Most severe primary headache
If repeated, occurs at same time of day or night
Men more affected.
Sharp stabbing intense pain lasting up to 3 hours
Can happen as often as 8x/day
Drug therapy for cluster headaches
If symptomatic:
Triptans- don’t use if pt has vascular disease
100% oxygen at 6-8L/min for 10 mins, can be repeated q5min
Preventative:
High dose verapamil
Invasive nerve blocks
Ablative neurosurgical procedures
