What are the differentials for headache? (x5 and x6)
A
VIVID – SINISTER CAUSES:
- Vascular – haemorrhage
- Infection – meningitis/encephalitis
- Vision-threatening – temporal arteritis, pituitary apoplexy, cavernous sinus thrombosis
- Intra-cranial pressure – space-occupying lesion, cerebral oedema, hydrocephalus, HTN
- Dissection – carotid dissection
NON-SINISTER CAUSES:
- Tension type
- Migraine
- Sinusitis
- TMJ dysfunction
- Cluster
- Medication withdrawal/overuse

What are the causes of space-occupying lesion?
A
Tumour or abscess. Tumour could be benign, malignant, primary or secondary.

Headache with third nerve palsy?
A
Sign before SAH from aneurysm. Third nerve palsy is due to compression of dilated vasculature. This is an emergency because it means that the aneurysm is about to pop.
