Types of Head Injuries
Primary- concussion, post-concussion syndrome, contusion, laceration
Secondary-damage from brain swelling (ischemia, ICP, cerebral edema, brain herniation)
Concussion
interruption of brain function WITHOUT loss of consciousness, recover in 24 hours
Post-concussion Syndrome
Headache, irritability, insomnia, poor concentration and memory may persist for months
Contusion
structural damage, bruising, seen on CT
Laceration
brain tissue is cut or torn
Focal Injury
focused localized portion of brain
contusion, laceration, hemorrhage
Diffuse Injury
affects the whole brain
concussion, contusion, axonal injury, hypoxic brain injury
Types of Skull Fractures (4)
- Simple
- Comminuted
- Depressed
- Basilar
Simple Fracture
linear break in bone
Complex Fracture
Splintered, multiple fracture line
Depressed Fracture
bone fragments embedded in brain tissue
Basilar Fracture
fracture of bones at base of skull; Ethmoid Cribiform Plate
Brain Injury Severity (3)
- Mild- short LOC, micro structural changes
- Moderate- longer LOC, hemiparesis, herniation, swelling
- Severe- coma, irreversible
Coup
1st hit, immediate damage at site of impact
Countre-Coup
2nd hit, rebound on opposite side of impact
