hand Flashcards
compartments of hand and muscles in each
thenar- Abduc pollicis br, FPB, oppones pol
Adductor- Add pol
Central- lumbricles, flex tendons
Hypothenar- Palmaris br, abduc digiti minimi, flex digity minii, oppones digiti minimi
interosseus- palmar + dorsal interossei
bands of palmar apponeurosis and what is superficial to it
4/5 longitudinal bands towards the fingers and will attach to skin of 4 medial digits, has longotundinal band at metacarpal pharangeal lvl
-palmaris longus located superficially
where does the ulnar nerve split up in teh hand
deep ulnar n is lat to pisiform and superficial ulnar n runs superficial to the peso hamate lig
if ulnar n is impinged distal in gunyons canal what would be affected
only deep (cuz superficial goes over)
lat and med border of gunyons canal
lat- hook of hamate
med- pisiform bone
what is the osseo fibrous tunnel and what is the floor and roof of it
-where the tendons of the palmar aspect run thru
floor- periosteum of th MC and phalangeal bones
roof- collagenous fibers that bridge over the tendons (arcuate and crutiate orientations)
what are the 2 layers of the osseofibrous tunnel
visceral synvial sheath around tendons of of FDP, FDS and parietal synovial sheath around whole thing
within the 2 layers of the osseofibrous tunnel what is there
vincula- holds artery and nerves
what are the 2 types of vincula
vincula brevia- short and at the level of tendons attachment
vincula longa- longer, one phalanx prior to tendons insersion
how far does each flexor sheath go (digits 1-5)
FPL and FDM extend to distal pahalanx
- if inflamation is at digit 2,3,4 the inflamation will be isolated at MCPJ
Path of ulnar a in hand
lat to ulnar n
- sends deep palmar br thru gunyuns canal
- after it will send superficial arch that meets with superficial radial arch (ulanr supplies 95%)
- common palmar digital arteries of 4 med digitits
Path of the radial a in hand (starting from before snuff box)
- before going to snuff box it sends a superficial branch that goes to superficial arch
- main branch goes to snuff box and comes back thru base of 1st metacarpal and makes deep branch arch
- also has 2 branches that go to thumb
what makes up dorsal carpal arch
from ulnar and radial arteries we create the dorsal carpal anastemosis
palm cutaneous nerves med to lat
superficial ulnar palmar cut of median palmar cut of ulnar median n palmar cut of superficial rad (base of thumb)
what is dupuytren contracture of palmar fascia
fibrous degen of palmar fascia and aponeurosis with formation of nodular thickenings attached to skin
(mostly bilat)
