Pathogenesis of Darier disease
Clinical features of Darier disease
1) peak onset in puberty
2) primary lesions - red brown keratotic crusted papule in a seborrheic distribution (trunk, scalp, face, lateral necl)
3) Can coalesce to form papillomatous masses
4) Small hypopigmented macules mixed with keratotic papules
5) Occasionally vesicles can ocxur - if prominent exclude herpes infection (kaposi varicelliform eruption)
6) Intertriginous lesions in most patients (axilla, groin, infra-mammary, can confuse with hailey hailey)
7) can become macerated with fungating masses and malodor
8) Brown flat-topped papules om dorsal hands/feet - resembles plane warts
9) acral haemorrhagic vesicles
10) Keratotic palmoplantar papules/depressions
11) Nail change
- wedge shaped subungual hyperkeratosis
-V-shaped notches
-longitudinal ridging
-longitudinal white/red lines
-brittle
12) painless white papules/ruggose plaques (hard palate, buccal muccosa, gingiva)
