tPA- Mechanism of action
activates plasminogen bound to fibrin (hence ltd to thrombus) converts it(plasminogen) to plasmin which reduces fibrin to fibrin split products.
How does tPA differs to Streptokinase
tPA naturally occurring human enyzyme
Streptokinase not an enzyme its a bacterial product which combines with plasminogen and catalyses conversion of further plasminogen to plasmin
prioir Strep infection can cause therapeutic resistance and allergic reaction due to preformed antibodies.
Erythropoietin- Mech of Action
Glycoprotein form kidney that stimulates red cell precursors to differentiate and proliferate
Also stimulates marrow to release reticulocytes
Erythropoietin- clinical use
Chronic anaemia form CRF.
Stimulates marrow in marrow failure states (aplastic anaemia, MPD, Cancer, AIDS)
Erythropoietin- toxicity
Rapid HB rise- HTN, Thrombosis
