Presentation of immune thrombocytopenia purpura?
Easy bruising
Epistaxis
Menorrhagia
Purpura (purple spots on the skin caused by bleeding underneath the skin)
Gum bleeding
Rare = splenomegaly, major haemorrhage
Investigating immune thrombocytopenia purpura?
Bone marrow exam - thrombocytopenia w/ increased or normal megakaryocytic in the marrow
Platelet autoantibodies - present in 60-70%
Managing immune thrombocytopenia purpura?
1st LINE
- corticosteroids (eg. Prednisolone)
- IV immunoglobulin (eg. IV IgG - raises platelet count more rapidly than steroids)
2nd LINE
- splenectomy
- then if fails, immunosuppression (eg. oral/IV azathioprine)
