Haemolytic Uraemic Syndrome (HUS) + TTP Flashcards
What is Microangiopathic haemolytic anaemia
Shearing of RBCs dur to microthrombi formation (nets) and leads to schistocytes
When does HUS present?
disease of early chidhood
What is the triad for HUS?
- Thrombocytopenia
- AKI
- MAHA: jaundice and conjunctival pallor
What can HUS be caused by?
E coli
What are some GI features of HUS?
abdominal pain and bloody diarrhoea
What are general symptoms and signs of HUS?
- Abdominal pain
- Anaemia with leucocytosis
- High serum bilirubin
- Thrombocytopenia
- AKI
What does TTP stand for?
Thrombotic Thrombocytopenic Purpura
What is the patho for TTP?
- ADAMTS-13 enzyme is dysfunctional and so webs can’t be broken down
2, Cleavage of the ADAMTS13 enzyme prevents build up of large oligomers (as shown on the right) which become microthrombi
What are symptoms of TTP?
- AKI
- MAHA
- Temperature
- Swinging CNS signs
(clinical pentad)
What does Ix show for TTP?
- Antiglobulin negative
2. Decreased platelets
How do you tell the difference between HUS and TTP?
HUS has decreased platelets, AKI and MAHA but TTP has additionally temperature and swinging CNS signs
What can plasmaphersis do?
removes antibodies against ADAMTS-13 and also replaces the enzyme
What does DIC, HUS and TTP have in common?
DAT/Combs negative
Is DAT and Coombs test the same?
yes
What does the DAT test tell you?
if there is an auto-immune problem directly attacking the red blood cells
