Myeloma/multiple myeloma investigation findings

A

Monoclonal proteins (usually IgG or IgA) in the seum and urine (as well as bence jones proteins.) MRI is the most useful test.

Myelofibrosis clinical features?

A

Myeloproliferative disorder caused by hyperplasia of megakaryocytes (the resultant release of platelet derived growth factor stimulates fibroblasts) Elederly person w/ symptoms of anaemia, massive splenomegaly and weight loss night sweats etc.

Myelofibrosis investigation findings?

A

anaemia, high WBC and Plt early in the disease, unobtainable bone marrow biopsy (therefore trephine biopsy needed) tear drop poikilocytes on blood film. High urate and LDH
