how should a normal bartholin’s gland feel?
- Pea sized
- Not palpable
what is the function of bartholin’s gland?
Function: produce mucus to help with vaginal lubrication
what is the pathophysiology of bartholin’s cyst?
Pathophys: when ducts become blocked, bartholin’s gland
- Glands can swell and become tender – causing cyst
- Swelling is typically unilateral and forms fluid filled cyst between 1-4cm
- Can become infected and form a Bartholin’s abscess
how would a bartholin’s abscess present?
: hot, tender, red and potentially draining pus
what is the conservative management of bartholin’s cyst?
Conservative: usually resolve with simple management such as good hygiene, warm compresses
- Incision is generally avoided as the cyst will often reoccur
when would a biopsy be indicated within bartholins cyst?
- Biopsy may be required if vulval malignancy over 40
how would you manage bartholin’s abscess?
Abscess: require Abx and swab of pus/ fluid from abscess and can be taken as culture of infective organism
what is the common cause of bartholins abscess?
e.coli
apart form cultures, what other investigation should be done with bartholin’s cyst?
- Send specific swab for chlamydia and gonorrhoea
what surgical management is available for bartholins cyst?
Surgical: may be needed fro abscess
- Word catheter: Bartholin’s gland balloon – needs LA
- Marsupialization- requires GA
