what is premenstrual syndrome?
occurs in the luteal phase and resolves by menstruation
what causes premenstrual syndrome?
decreased progesterone
increased PRL/oestrogens
features of premenstrual syndrome?
emotional lability bloating wt gain mastalgia fatigue headache cravings cyclical
features of a molar pregnancy?
missed period
vaginal bleeding
large for dates
what causes a molar pregnancy?
non viable fertilised egg implants into the uterus/fallopian tube
what is a partial mole?
egg fertilised by one/two sperm (triploidy)
what is a complete mole? what are these at risk of?
egg with no DNA fertilised by sperm (diploidy)
choriocarcinoma
more common (snowstorm etc..)
Ix for molar pregnancy?
BhCG elevated pelvic US (snowstorm) CXR (metastatic disease)
Tx for molar pregnancy? how long should pregnancy be avoided?
D+removal / hysterectomy
anti D
avoid pregnancy for 1 year
what is the menopause?
amenorrhoea for 12 months
when is the menopause premature?
<45yrs
features of the menopause?
hot flush night sweats joint pain dry sin hair thin vaginal dryness osteoporosis poor memory loss of libido labile mood/concentration
what happens to cause the menopause?
decline in follicles
reduced oestrogen (which is produced by these)
no negative feedback on LH/FSH production
anovulation occurs
what does lack of oestrogen increase risk of?
CVS
osteoporosis
incontinence
prolapse
Ix for menopause?
FSH (high)
how long should women use contraception for after the menopause?
<50 2 years after LMP
>50 1 year after LMP
