Intro
B4 choosing antibiotic consider:
- PT (M/F, age, allergy etc)
- Causative organism
- Risk of resistance with repeated course (more risk of treatment failure) [Ideally use short period with lower dose]
Pt Factors
Allergy status
Renal and hepatic function
Susceptibility to infection (immunocompromised)
Ability to tolerate drugs by mouth
Severity of illness
Ethnic origin
Age, Other meds, Female,
Pregnant, Breastfeeding
Take oral contraceptive?
Rational for prescribing
B4 starting:
- Viral? - NO ANTIBITOICS
- Samples - AVOID blind treatment
- Knowledge of prevalent organism (narrow vs broad spec?) - Narrow preferred ideally but in practise broad spec used
- Dose adjustments (Weight, Renal impairment etc)
- Route of admin. AVOID painful IM in child
- Duration of treatment - Prolong use = more Resistance risk
