gout vs pseudogout
gout: uric acid chrystals, tophi, rat-bite on xray
pseudogout: effects larger joints, calcium pyrophosphate cyrstals, chondrocalcinosis on xray
first line therapy for gout
NSAIDs
- if at risk for side effects from NSAIDs, give additional PPI

Indomethacin / Indocin
class: NSAID
indication: gout, arthritis, bursitis, migraine, pain and swelling, close patent ductus arteriosus
contraindications: pt with nasal polyps can develop angioedema, infants under 2 unless it is to close patent ductus arteriosus, pregnancy, nursing mothers
Samter’s Triad
- asthma
- aspirin/NSAID sensitivity
- nasal polyposis
colchicine
class: mitotic inhibitor
indication: gout
side effects: GI upset, anemia, neutropenia, hair loss, peripheral neuropathy
*can have toxic reaction
allopurinol
class: purine analog, xanthine oxidase inhibitor
indication: hyperuricemia, gout, prophylaxis against tumor lysis syndrome
SEs: N/V, skin rash, SJS, hepatitis
probenecid
class: uricosuric
indication: hyperuricemia, gout
SEs: N/V, gastritis, GI upset, renal stones
