What are the 4 drugs given to treat peptic ulcers?
NSAIDS
Paracetamol
PPI-omeprazole and lansoprazole
Histamine 2 receptor antagonists- ranitidine
What is the action mechanism of NSAIDS?
Inhibit cyclo-oxygenase which is the rate limiting step for production of prostaglandin
Prostaglandin bind to receptors that cause nociceptors mediators to cause pain
What are the side effects of NSAIDS?
Gastric irritation and ulceration
Skin rash
Hypertension
Renal failure after long term use
Why do prostaglandin inhibition increase chances of ulcers?
Protect from acid by increasing bicarbonate release and mucus production in gastric mucosal cells
What is the action mechanism of PPI?
Inhibit H+/K+ATPase pump in gastric parietal cells
Inhibit gastric secretion by 90%
What are the side effects of PPI?
Diarrhoea
Headache
Bloating
Abdo pain
What is the action mechanism of h2 receptor antagonists?
Competitive antagonists of receptors
No histamine released from enterochromaffin like cells
Inhibit gastric acid secretion by 60%
Why would a patient with osteoporosis be given h2 antagonist rather than PPI?
PPI decreases amount of calcium digested (needed for bones)
