what is gonorrohoea?
Gonorrhoea is a sexually transmitted infection (STI) caused by the gram-negative diplococcus, Neisseria gonorrhoeae.
who is gonorrhoea seen in?
15–24 years
RF for gonorrohoea?
Young adults
New sexual contacts
Inconsistent barrier contraception
Men who have sex with men
Previous STI
Deprivation
features of gonorrhoea?
ix for gonorrhoea?
Self-taken vulvovaginal swab in women or self-obtained first pass urine in men; self-obtained rectal swab; or clinician-obtained endocervical or penile swab
Microscopy revealing monomorphic Gram-negative diplococci within polymorphonuclear leukocytes
Nucleic acid amplification tests (NAAT)
Culture & sensitivities
mx for gonorrhoea?
admit if systemically unwell
Ceftriaxone - single IM injection
alternative = intramuscular gentamicin or oral cefixime, both with azithromycin orally
what is given to pregnant or breastfeeding women with gonorrohoea?
a single dose of intramuscular ceftriaxone or oral azithromycin should be used.
what is essential following tx in gonorrohea?
For all patients following treatment, a test of cure is essential to monitor disease clearance and assess the effectiveness of the chosen antibiotic regimen.
complications for gonorrhoea?
Infertility in women due to pelvic inflammatory disease (PID), which may result in scarring of the tubes and an increased risk of pregnancy complications
Infertility in men due to epididymitis caused by the spread of gonorrhoea to the epididymis
Disseminated infection that may affect joints and other areas of the body, leading to fever, rash, skin sores, joint pain, swelling and stiffness
Increased susceptibility to human immunodeficiency virus (HIV) infection, potentially leading to AIDS, and enhanced transmission of both diseases to partners
