sulfonylureas consideration (Beers Criteria)
-higher risk of CV events & all cause mortality
-higher risk of hypoglycemia
-MAY increase risk of ischemic stroke & CV death
-use short-term over long-term (glipizide over glyburide)
Insulin sliding scale considerations (Beers Criteria)
-higher risk of hypoglycemia
-if using bolus use basal insulin as well
Physiologic changes in geriatrics
-DECREASE TBW, lean body mass, baroreceptor response/activity, heart rate variability, hepatic blood flow, neurotransmitter volume
-INCREASE body fat
decrease in VD
increase in concentration of water-soluble drugs
-atenolol
increase in VD
increase in half-life of lipid-soluble drugs
-rifampin
decrease clearance
increase in half-life of hepatically & renally cleared drugs
-propranolol & atenolol
AE of anticholinergic/antimuscarinics
-dry mouth, constipation, confusion
anticholinergic/antimuscarinic medications
-oxybutynin (Ditropan, Oxytrol, Gelnique)
B3 agonist medications
-mirabegron (Mybetriq); Vibegron (Gemtesta)
B3 agonist AE’s
minor increase in BP, UTI
