Domains: Domains are how they assess the quality of care provided in healthcare: Skin integrity, functional status, medication reconciliation, incidence of falls, transfer of info, resource use, discharge to the community, risk of readmission.

What criteria must a home health agency (HHA) meet in order to meet eligibility requirements from Medicare?

A
  • The person that is receiving services is a Medicare beneficiary, the home health agency participates in the Medicare Program, beneficiary is qualified for home health services, services that are given are covered, Medicare is the appropriate source of payment, services are not excluded.
  • The agency has to be Medicare compliant, meet eligibility criteria in order to qualify.

What criteria must a Medicare beneficiary meet in order to qualify for home health services?

A

-Care is under a MD, the MD must provide certification for intermittent: skilled nursing care, PT, SLP, continued OT, MD has to certify that the patient is homebound.

What does “being homebound” mean?

A

-Being homebound means that the individual is unable to leave their house because of illness or injury. Certified homebound by a physician. Having extreme difficulties leaving the house. Homebound exceptions: funeral, wedding, graduation, healthcare visits, attend an adult day care center, religious ceremonies (i.e. church).

What are the regulations that impact therapists in home health-care (Specifically look at plan of care POC, initial visit, comprehensive assessment and OASIS)?

A

-POC: the course of treatment has to be established by the doctor, therapy has to be in agreement with the POC

  • Initial visit (OASIS) and Comprehensive assessment: For rehab ONLY PT and SLP can open the case, all disciplines involved must contribute to show a comprehensive overview of the client, assessment of medications and medication management is a requirement for all disciplines. *48 hours initial visit
  • Nursing can also open a case
  • OASIS: assessment points required to be collected at various times, standard for individual’s function, mandated that this assessment is used, what allows reporting of quality assurance measures, able to reflect the amount of progress the client is making
  • Items should be considered throughout the assessment

  • Initial visit: PT or SLP

- SOC comprehensive assessment: any discipline that is qualified and involved in care will contribute to the report

After the initial visit, can OT be the only service that stays in?

A

-All of the disciplines involved should be a part of the comprehensive assessment in order to get the best overall picture of the client’s status and function. Only if OT services were introduced before the qualified services were introduced we can stay in. As long as OT evals if the other disciplines have not d/c we can remain in the service.

What qualifies equipment as durable medical equipment (DME)?

A

-Equipment that is qualified as DME is, durable, has a medical reason for use, not useful for someone that is NOT sick, injured, or does NOT have a disability, equipment that is used in the individual’s home, and should withstand normal use for at least three years.
