Patient is clean and smells good, Shaved, has makeup on.
Dress
Appropriate for weather, setting, age, culture, clean.
Affect (Objective)
How their mood appears to others
Mood (Subjective)
How they describe their feelings
Behavior
Cooperative, friendly, hostile, angry.
Speech
Can form words (articulation), clear, normal volume.
Height, Weight, and Vitals
Assessed after the General Survey
Monitor for weight loss
Collect Daily Weight
Height- No shoes and supine position
