- SS causes hyperreflexia and clonus (which you don’t get in NMS)
- NMS causes muscle rigidity (lead pipe rigidity) which is less common in SS (get more myoclonus)
- NMS commonly causes high CK
- NMS due to antipsychotics, SS more often due to antidepressants
