presentation of Ovarian cancer
- often presents late (60% late stage at diagnosis)
- non-specific presentation
–ascites/ bloating
–pelvic mass/ bladder dysfunction
–pleural effusion/shortness of breath
–incidental finding
B for bloating that is persistent and doesn’t come and go
E for eating less and feeling fuller
A for abdominal pain
T for telling your GP
what ultrasound features suggest ovarian Ca?
US features
- Multilocular cyst
- Solid areas
- Bilateral lesions
- Ascites
- Intra-abdominal mets
how is RMI (relative malignant index) calculated for ovarian ca?
At what level should the be referred to gynae
RMI score = ultrasound score x menopausal score x CA 125 level
refer if RMI>200
Describe the staging for ovarian cancer
I- confined to 1 or both ovaries
II-spread to other pelvic organs eg uterus, fallopian tubes
III- spread beyond the pelvis within the abdomen
IV- spread into other organs eg liver, lungs

what is chemotherapy for advanced ovarian cancer?
First line chemotherapy treatment of epithelial ovarian cancer should
include a platinum agent either in combination or as a single agent,
unless specifically contraindicated.
Carboplatin is the platinum drug of choice in both single and
combination therapy.
Paclitaxel is recommended in combination therapy with platinum
in the first line post-surgery treatment of epithelial ovarian cancer
where the potential benefits justify the toxicity of the therapy. In
those unable to tolerate paclitaxel, peglated liposomal doxorubicin
or gemcitabine in combination with carboplatin can be used as an
alternative.”
what chemotherapy is offered for early stage ovarina ca?
All women with high-grade early stage (Ia-Ib) ovarian cancer should
be considered for adjuvant chemotherapy.
For early stage disease, maintenance cytotoxic chemotherapy should
not be given
