Symptoms typically develop suddenly, starting with epigastric pain that later becomes more generalized. Patients may also describe syncope.

Plain x-rays are recommended as the first form of imaging to obtain.
What type of chest x-ray is usually required when a patient presents with acute upper abdominal pain?
An upright (‘erect’) chest x-ray is usually required.
Why is an upright chest x-ray useful in cases of suspected perforated peptic ulcer?
It is useful because approximately 75% of patients with a perforated peptic ulcer will have free air under the diaphragm.
