gastric acid and pancreatitis lab Flashcards
two most common causes of pancreatitis
alcohol
gall stones
acute pancreatitis is characterized by..
the destruction of the pancreas due to the autodigestion of pancreatic tissue by inappropriately activated pancreatic enzymes
what is the mnemonic for the causes of pancreatis
I GET SMASHED
idiopathic, gallstones, ethanol, trauma, steroids, mumps, autoimmune, scorpion bite, hypercalcemia/hyperlipidaemia, post-ERCP, drugs
what are the components of the gastric juice
salt, water, HCL, pepsin, IF, mucous
explain the steps for the production of HCl
- water in and CO2 in –> H2CO3 –> H+ + HCO3-
- H+ and K+ cotransported across the luminal membrane (H+ out and K+ in)
- K+ transported back out into the lumen
- HCO3 and Cl- cotransported at interstitial membrane (HCO3 into interstitium –> blood and Cl into cell)
- Cl transported into lumen
what is the mechanism of histamine stimulating HCl secretion
binds to H2 R on basement membrane –> AC converts ATP to cAMP. –> PKA stimulates the release of HCl
what is the mechanism for ACh stimulating HCl release
binds to M3 R on basal membrane of parietal cell –> initiates Ca influx –> Ca binds to CAM –> stimulates HCl secretion
what is the effect of caffeine on HCl production and how does this work
increases HCl secretion as it blocks phosphodiesterase breaking down cAMP to AMP so therefore more cAMP to activate PKA for HCl secretion
