enteric Gram - bacteria move up tree
Gallbladder carcinoma
usually porcelain gallbladder in old lady
presents as cholecystitis
Gallstone ileus
fistula between gallbladder and duodenum
large stones then obstruct the duodenum
Biliary atresia presents
in first few months
w/ elevated conjugated bili
Gallstone types
cholesterol or pigment (bilirubin)
Biliary colic
waxing and waning RUQ pain
due to gallbladder contraction against stone
Acute cholecystitis
etiology
acute inflammation of galbladder wall
dilation –> pressure ischemia

RUQ pain radiating to right scapula
elevated alkaline phosphatase
Chronic cholecystitis
pathology findings
Rokitansky-Aschoff sinuses
invasion of mucosa into sm muscle layer
