Anesthetics esters vs amides
Esters: cocaine, tetracaine, proparacaine, procaine, benoxinate
Amides: bupivicaine, lidocaine, mepivicaine
Not necessarily allergic cross-reactivity
Alpha one adrenergic receptors
Smooth muscle contraction (arteries, decrease aqueous production by reducing ciliary body blood flow, iris dilator, miller’s muscle
Alpha 2 adrenergic receptor
Feedback inhibition, ciliary body (decreases production and /or increase his outflow)
Beta one adrenergic receptor
Cardiac stimulation
Beta two adrenergic receptor
Pulmonary and Gastrointestinal intestinal smooth muscle relaxation, cilary body / TM (increase aqueous production, increase outflow facility)
Direct acting cholinergic agonists
Acetylcholine (miochol), pilocarpine
Muscarinic (anticholinergics)
Atropine (1 to 2 week duration)
Scopolamine (One week duration), home atropine (1 to 3 day duration), cyclopentolate (24 hours duration),
tropicamide (4 to 6 hour duration)
Direct acting alpha agonists
Epinephrine, phenylephrine, dipivefrin, Clonidine, apraclonidine, brimonidine, methyldopa, naphazoline, oxymetazoline, tetrahydrozoline
