Fuoroquinolones
FQ mechanism?
CIDAL - inhibit bacterial DNA gyrase & topoisomerase
Resistance to FQ?
plasmid-mediated: Qnr proteins that protect DNA gyrase & acetyltransferases that modify cipro
FQ absorption?
PO (also IV)
FQ distribution?
well - especially into lungs & high urinary levels
FQ elimination
primarly by kidney
Cipro Spectrum?
G- (Pseudomonas) good for UTIs
Levo Spectrum?
G+/- (strep, MSSA, pseudo) good for lung infections
ADRs to FQs?
GI (CDAD), CNS disturbance, tendon rupture, arthropathies
DDIs w FQs?
Theophylline (decreased metabolism), antacids (reduced po absorption)
Nitrofurantoin mechanism?
static/cidal - damage DNA (needs to be reduced in bacterial cell)
Nitrofurantoin absorption?
PO (rapid GI abs)
Nitrofurantoin metabolism?
rapid urine excretion (urinary antiseptic)
Nitrofurantoin spectrum?
G- (E coli resistant to TMP/SMX) for UTIs
ADRs to nitrofurantoin
GI upset, hypersensitivity rxns (hemolytic anemia, hepatocellular damage), neuropathies, Cat B in pregnancy
