State Wagner Classification
Describe the general principle of treatment for DFU
- Elimination of infection
- Debridement
- Antibiotic
- Wound care - Reduction of plantar pressure (off-loading)
- Total contact plaster cast
- Non-weight-bearing crutches - Vascular management
- Angioplasty or bypass - Medical management
- Good diabetic control - Reduce risk of recurrence (Foot wear and foot care)
Define Charcot joint
A chronic, progressive, degenerative, neuropathic arthropathy resulting from loss of protective sensation
List 4 indication of amputation
- Dead/dying (e.g gangrenous leg or traumatic injury)
- Deformed/dysfunctional. Deformed leg with loss of sensation
- Deadly (e.g osteosarcoma or infection (septic)
- Damn nuisance (e.g large smelly ulcer/wound)
Define hallux valgus
The 1st metatarsal deviates medially to a variable degree
The great toe deviates laterally and may be rotated into pronation
Define Hammer toe
Flexion deformity affecting PIPJ + Extension of MTPJ
Define Claw toe
Dorsiflexion of the proximal phalanx on lesser metatarsophalangeal joint, combined with flexion of both PIPJ and DIPJ
Define Mallet toe
Flexion deformity affecting DIPJ
Define Plantar fasciitis
Inflammation of the aponeurosis of the plantar fascia at its origin on the calcaneus
What are the findings of plantar fasciitis on physical examination?
- Tenderness at medial tuberosity of calcaneus
- Gastrocnemius tightness (limited ankle dorsiflexion)
What are the 2 mechanism for Achilles tendon rupture
- Trauma during sports event
- Sudden forced plantar flexion
- Violent dorsiflexion in a plantar flexed foot - Repeated steroid injection
