What are the stages of FTSG healing?
- Imbibition (24-48 hrs), simple plasma imbibition
- Inosculation (48 hrs to 5 days), capillary buds, early blood flow, deposition of fibroblasts and collagen
- Revascularization, 5+ days
What is inosculation?
The second phase of vascularization of a full thickness skin graft. Occurs from 48 hours to five days and includes ingrowth of capillary buds, early blood flow and deposition of fibroblasts and collagen
Describe how a wound VAC works?
- Macro strain
Close wound edges together, removes exudate. - Micro strain (micro deformation at the cellular level which leads to sell stretch)
Reduces edema, promotes perfusion, promotes granulation by facilitating cell migration and proliferation
Tell me about Z-plasties and their theoretical lengthening potential?
45°: 50% increase in length
60°: 75% increase in length
Combining multiple Z-plasty’s is additive (theoretically)
What is the pedicle for the groin flap?
The superficial circumflex iliac artery which is a branch of the femoral artery. It runs 2 to 3 cm inferior and parallel to the inguinal ligament, superficial to the fascia. It divides into superficial and deep branches at the medial border of the sartorius muscle.
The anterolateral thigh flap is based on which artery?
