Assess pt to determine if they have one or more of the following physiological criteria:
a. pt doesn’t follow commands
b. systolic bp <90mmHg, or
c. Resp rate <10 or >30 bpm or need for ventilatory support (,20 in infant aged <1 yr)
If pt meets physiological criteria AND the land time is <30 mins to LTH…
Transport the pt directly to the LTH or equivalent hospital
If pt does not meet physiological criteria and land transport time, assess for anatomical criteria…
a. Penetrating injuries to head, neck, torso and extremities proximal to elbow & knee
b. Chest wall instability or deformity
c. Two or more proximal long-bone fractures
d. Crush, de-gloved, mangled or pulseless extremity
e. Amputation proximal to wrist or ankle
f. Pelvic fracture
g. Open or depressed skull fracture; or
h. paralysis
If pt meets anatomical criteria and land transport time is estimated <30 mins to LTH
Transport to LTH or equivalent hospital
What do you do if unable to secure airway or survivial to LTH is unlikely?
Transport pt to the closest ER
Despite airway or unlikely to survive, transport to LTH if pt has penetrating object to head, neck, torso and meets ALL…
a. VSA yet not subject to TOR described in directive
b. Land transport to the LTH is estimated to be <30 mins
If pt doesn’t meet physiological or anatomical, use MOI to determine…
a. Falls (adults >6 meters, children ( 2 or 3 times the height of child))
b. MVC (intrusion, ejection, death in same compartment)
c. Pedstrian thrown, run over or struck (>30km/hr)
If pt meets MOI AND land transport time is estimated <30 mins
- Determine the need for pt transport to LTH
What is the special criteria?
a. Age (Death increase after 55, SBP <110 represent shock after 65)
b. anticoagulation and bleeding disorders
c. burns (w trauma mechanism
d. pregnancy >20 weeks
