Does our loader needs to be engaged into the shealth completely?
Yes. If not fully in, the force from outside may cause the AVP wire to bend if not careful.
But in Dr Raj case, Seto did not fully engage the loader as it was too tight. But it was still possible to push the AVP in despite loader not completely engaged.
Why the blood keep coming out of the after we remove the stopper?
Because the was nothing stoping the blood flow. The head was too right and they had difficulty inserting the avp delivery wire.
In this case. A touhey is needed. To prevent the back flow as well is allow the wire to be inserted smoothly.
What is the meaning of Constantine “can we Constantine the plug?”
Means to pack it. In Dr Raj case, what I could have suggested is : pull back the plug, shift the sheath further in. Redeploy.
Do we need exact French size as indicated by the package?
No. That’s the minimum. But what’s more important is to look at the mm size on the packaging of the sheath or guide catheter before them using.
Can any size of the shealth be used as long as it fulfills the minimum?
No, one size bigger is fine, not too big as it will deploy first if the shealth is too big.
