What is fibrocystic change
A
Non-cancerous breast lumps

Who is affected
A
50% women of child-bearing age

What is the cause
A
Unknown –> oestrogen probably cause
- Subsides after menopause

What are some risk factors
A
- Iodine deficiency –> enhances breast sensitivity to oestrogen

How does it present
A
- Discomfort – usually in relation to hormonal menstrual cycle changes – symptoms peak before period
- Rubbery, well-circumscribed, mobile mass (breast mice)
- Usually confined within the upper outer edge of breast
- Tender, itchy nipple sometimes green/brown non-bloody discharge
- Lumps can blend into surrounding tissue
- Pain/tenderness
- Lumps fluctuate size w/ menstrual cycle

What are some DDx
A
- Fibro-adenoma
- Fat necrosis
- Intra-ductal papilloma
- Breast abscess
- ADH + ALH
- Breast cancer
- DCIS

What investigations do you perform
A
Clinical breast exam
Breast ultrasound
–> Simple cyst = internal echoes, complex cyst = cystic + solid components
–> distinguish fluid filled + solid mass
Mammography
–> cannot distinguish between cystic + solid mass
Fine needle aspiration/biopsy

What is the complication
A
- may slightly increase risk breast cancer
- cancerous tumour detection more difficult

Q
How do you treat fibrocystic change
A
- If asymptomatic - no Tx
- Fine needle aspiration –> drain fluid (confirms cyst) + collapses
- Surgical excision
