What is ferric carboxymaltose?
An iron carbohydrate complex containing 5% (50mg/L) of iron designed to provide iron for the iron transport and storage proteins in the body.

for adult patients who have intolerance to oral iron or have had unsatisfactory response to oral iron or those who have non-dialysis dependent chronic kidney disease.
What is the dose of ferric carboxymaltose given in iron-deficiency anemia?
calculated according to bodyweight and iron deficit.
What is the mechanism of action of ferric carboxymaltose?
Ferric carboxymaltose solution contains iron in a stable ferric state. It provides the iron required for iron transport and iron storage proteins (transferrin and ferritin) in the body.
FCM increases Hb and serum ferritin concentrations in patients with mild-mod iron deficiency anemia.
What are the main PK and PD parameters with ferric carboxymaltose use?
PK:
Mean volume of distribution: 3L
Terminal half life: 7-12 hours
Renal elimination: negligible.
Iron is incorporated into blood cells within 6-9 days.
What are the precautions associated with ferric carboxymaltose use?
ionoverload/haemosiderosis- monitor patient RBC and serum ferritin regularly to detect iron overload.
Patients with liver dysfunction: only admin if benefit > risk
Patients with infections: Use with caution in acute/chronic infection, asthma, eczema or atopic allergies. Recommended to stop treatment in patients with ongoing bacteraemia.

What are the adverse effects with ferric carboxymaltose use?
common:
headache, dizziness, hypertension, nausea, abdominal pain, constipation, diarrhoea, injection site reactions, increased ALT and AST levels, hypophosphataemia
What are the drug interactions with ferric carboxymaltose use?
absorption of oral iron is reduced when administered concomitantly. Do not start oral iron therapy for at least 5 days after the last injection of ferric carboxymaltose
What are the alarm bells associated with ferric carboxymaltose use?
Be aware of iron overload and hypophophataemia
What monitoring is involved with ferric carboxymaltose use?
Hb and serum ferritin levels
Phosphate levels
